feat(alerts): escalation chains that page step by step until someone acknowledges - #2304
Chains (escalation_chains): ordered steps, each "notify these destinations, then wait N minutes for an ack", the whole chain repeated repeat_count more times.
Rule tiers can name a chain (escalationChainId). The tier's own destinations still fire right away and the chain runs on top. A tier may also have only a chain.
Runs (alert_escalation_runs): one per incident. A run starts in the same transaction that opens the incident or escalates it to a tier with its own chain. It snapshots the chain and the trigger payload, so editing a chain cannot shift a page that is already running.
Timer: the standard alert lane advances due runs between rule evaluation and the delivery drain, so a due step is sent in the same tick (at most about 60s late, recorded as maple.escalation.lateness_ms). This reuses the existing minute cron and the alert_delivery_events outbox, so steps get the existing retries, delivery log and timeline. A Durable Object alarm would have added a second state store and a cancel path on every ack, and alarms fire at least once anyway.
At most once per step: each advance is a compare-and-set on the run's position, committed with the step's delivery rows. The step also locks its incident row, so a resolve or ack that commits first wins. The drain drops a queued step whose incident has since closed or been acknowledged.
Stops and changes:

v2:/v2/alerts/escalation_chains (CRUD, esc_ ids). Rule tiers carry escalation_chain_id. A PATCH tier that omits the field keeps the stored chain; only an explicit null detaches it.
MCP:list_escalation_chains, create_escalation_chain, update_escalation_chain, delete_escalation_chain, and escalation_chain_id on the create and update rule tools.
Guards (under the same per-org advisory lock as rule writes):
a rule naming an unknown chain is a 400;
deleting a chain a rule uses is a 409;
deleting a destination a chain step uses is a 409.
Unchanged:issue_escalation_policies / EscalationService (one-shot triage severity routing, no timer).
No new event types: steps are delivered as trigger, because the iOS app decodes AlertEventType as a closed list.
Verification
New tests in AlertsService.escalation.test.ts, 12 cases against PGlite:
walk the chain;
repeat it;
stop on ack and resume on unack;
end on resolve;
a second writer at the same position loses its compare-and-set;
acknowledged runs are not read;
the drain drops a step after an ack;
continue after a flap-suppressed re-open;
stop on de-escalation;
restart on escalation to a chained tier;
deduplicate against the tier's own trigger;
the delete and validation guards.
Step planner:alerting-core table tests (11).

Summary by CodeRabbit
New Features
Create, view, update, and delete escalation chains through the API and AI tools. Chains send notifications to selected destinations in sequence, with configurable wait times and repeats.
Assign escalation chains to alert-rule severity tiers. Existing destinations continue to notify immediately, alongside the chain.
Escalations advance over time, pause when an incident is acknowledged, and stop when it is resolved.
Bug Fixes
Alert-rule updates preserve an assigned chain when the chain field is omitted. Deletion is blocked when a chain or active escalation uses a destination.
An open incident can now be acknowledged ("someone is on it") and
unacknowledged. The ack is a sub-state of `open`, like the telemetry hold:
four nullable columns on alert_incidents (who, when, name, source), so every
open/resolved consumer keeps working. alert_incident_events keeps the history
(acknowledged, unacknowledged, ack_cleared) for the timeline and for on-call
escalation to read.
The lifecycle planner holds reminders and de-escalation notices while an
incident is acknowledged. An escalation to a higher severity still pages and
clears the ack. Resolve still sends the all-clear and clears it.
Surfaces: POST /v2/alerts/incidents/:id/acknowledge and /unacknowledge
(alerts:write, 409 on a resolved incident), GET /:id/events, an
acknowledged=true|false list filter, and the acknowledge_alert_incident MCP
tool (approval-gated in chat like the other mutating tools).
… in SQL
The escalation cleared the ack from the tick's prefetch and wrote ack_cleared
after the claimed update, outside it. A failed event insert could then lose
the escalation page, and a human ack or unack during the evaluation got the
wrong outcome. Every escalation now clears whatever ack stands when it
commits, with a guarded update in the same claimed write, and writes
ack_cleared only when one was cleared. The planner no longer carries the
flag.
Also: an API-key ack records no user (the key is not a person), the
unacknowledge docs say an overdue reminder goes out on the next check, and
the MCP incident list takes an acknowledged filter.
… and lose to a resolve or ack
- the scheduler reads only runs with a decision to make, so standing acks cannot crowd out due steps
- a step locks its incident and the drain drops a step whose incident closed or was acknowledged
- a flap-suppressed re-open starts the chain past step 1 instead of dropping it
- de-escalation stops the critical tier's chain; escalation refreshes a running chain's payload
- v2 PATCH tiers without escalation_chain_id keep the stored chain; MCP "none" detaches every tier

This change adds escalation-chain storage, management APIs and MCP tools, alert-rule tier associations, and scheduled escalation processing. It also updates alert-rule forms, validation, and public API contracts to carry escalation-chain IDs.
Adds escalation-chain and run schemas, branded IDs, database tables, migration indexes, audit labels, and public API contract coverage.
Chain management service and HTTP API packages/backend/src/services/alerts/EscalationChainsService.ts, apps/api/src/routes/v2/escalation-chains.http.ts, apps/api/src/routes/v2/alert-rules.http.ts, apps/api/src/runtime/*, packages/backend/src/services/alerts/AlertDestinationsService.ts, packages/backend/src/services/org/OrganizationService.ts, apps/api/src/routes/v2/alerts.http.test.ts
Adds tenant-scoped chain CRUD, destination and rule-reference checks, audit events, API routing, and organization purge registration. Alert-rule PATCHes preserve a tier’s chain ID when that field is omitted.
Carries chain IDs through rule thresholds and form state. Validation accepts a chain when a tier has no destinations, and rule updates can retain or change tier assignments.
Run planning and delivery packages/alerting-core/src/escalation.ts, packages/backend/src/services/alerts/alert-escalation-runs.ts, packages/backend/src/services/alerts/AlertsService.ts, packages/backend/src/services/alerts/alert-delivery-rows.ts, packages/backend/src/services/alerts/delivery/stored-payload.ts, packages/alerting-core/src/escalation.test.ts, packages/backend/src/services/alerts/AlertsService.escalation.test.ts
Adds escalation scheduling and persistence. Incident events start, refresh, or end runs; the scheduler advances due steps, observes acknowledgement and silence state, and queues eligible deliveries.
Adds MCP tools to list, create, update, and delete chains. Alert-rule MCP tools can set and display chain assignments, with output schemas, registration, guidance, metadata, and tests.

Migration check (20261106120821_escalation_chains): additive only. It creates two new tables (escalation_chains, alert_escalation_runs) with their indexes and one FK to alert_incidents (on delete cascade). It does not alter or backfill any existing table, so it is safe on prod data and with the current code still deployed (old code never reads the new tables; tier JSON with an extra optional escalationChainId decodes on old code too).

Assert the escalation array bounds in the OpenAPI test.
The generated full v2 document should expose minItems: 1 and maxItems: 10 for both steps and targets. The HTTP test covers runtime rejection, but it does not detect a regression that removes these bounds from the public OpenAPI contract.

A due step first asks whether a silence covers the incident (one rule read,
only when a step would go out) and defers under one, rechecking every minute.
An incident that opens under a silence still starts its chain; step 1 waits.
When the silence ends, the catch-up page re-keys the chain's due step to its
own time, so a destination in both the tier and the step is paged once.
