Summary

Full execution of Phases 0–3 of the audit-and-fix brief: read the codebase end to end, ran a truth audit against a fresh install, produced a prioritized defect report, then fixed every CRITICAL and HIGH finding plus four MEDIUMs — each fix paired with a regression test.

Test suite: 20 → 185. Lint, types, import walk and boot are all clean.

Bugs fixed

CRITICAL

ID Defect Fix
C1 Total provider failure returned 200 / SUCCESS / confidence 0.92 with canned evidence and total_tokens:120 Degraded status propagates to every execution route; confidence 0.0, real error surfaced, degraded results never cached
C2 rotate_tenant_key accepted any old_key (including empty) → 200 ROTATED and unbounded attacker-controlled key minting old_key validated against active keys → 403; new keys from secrets.token_hex(8); superseded key retired so active_keys stays bounded
C3 114 of 120 operations accepted anonymous callers; X-FRIDAY-API-Key was declared on 3 endpoints but never read One fail-closed dependency applied router-wide: no server key → 503, missing → 401, wrong → 403 (hmac.compare_digest). 114/120 → 4 public (health probes, /agents, dashboard)
C4 /v1/governance/prometheus-metrics returned 7 hardcoded series (412/1240/88 requests, all providers health=1) as JSON a scraper could not read Real Prometheus text built live from monitor, concurrency controller, telemetry cache and circuit breakers; provider series appear only once real calls exist

HIGH

ID Defect Fix
H5 Debate returned "[Specialist X temporarily offline]" prose with constant 0.90 confidence Failed specialists raise → peer candidates retry with coverage recorded; all-dark panel → 503; confidence earned from extracted claims/evidence
H6 Trading consult recorded failed runs as status="completed", claimed "Multi-Agent Deliberation" from canned text, flat 0.88 confidence Honest run records, participation-driven confidence (0.45–0.88), [FALLBACK] markers, degraded decisions not cached
H7 Both rate limiters keyed on request.client.host: proxy collapses everyone into one bucket; "pytest" in sys.modules + loopback disabled limits entirely resolve_client_ip trusts X-Forwarded-For only from loopback/trusted CIDRs; bypass requires explicit dev flag; .env.example no longer ships APP_ENV="development"; malformed Content-Length no longer 500s
H8 Naive datetime.utcnow() mixed with aware timezone.utc → proven TypeError on compare/subtract All 16 call sites aware UTC; SQLite normalises legacy naive rows on read; AST guard prevents reintroduction

MEDIUM

M9 unreachable duplicate modules removed · M12 credential detection now covers all 9 patterns and verifies after scrubbing (refuses 500 rather than forward a secret) · M14 pytest config consolidated (asyncio_mode=auto was being silently dropped) · M15 three caller-keyed process-global maps bounded · M13 compose stack rebuilt so it can actually start.

Tests added — 11 suites, 165 new tests

Verification (executed, not asserted)

ruff check . → All checks passed!
mypy app → Success: no issues found in 231 source files
pytest tests/ → 185 passed in 2.13s
import walk (pkgutil) → 185 modules, 0 failed
from app.main import app → APP OK, 120 operations

Live server proof against a booted instance:

anonymous → 401 on /ask, /v1/task/execute, /v1/admin/dashboard,
             rotate-key, /v1/friday/ask, /v1/forge/generate-code
wrong key → 403 Forbidden: invalid API key
public     → 200 on /health /agents /ui / /health/detailed /health/providers /status
valid key → 200 on governance, admin, analytics; 404 on unknown task
rotation → 403 wrong/empty old_key, 401 anon, 200 valid; active_keys 1 → 1

Caveats

  • Docker: not installed in the audit sandbox. docker-compose.production.yml is validated structurally by 11 tests (6 of which fail against the previous file), but docker compose up was not executed here.
  • Provider calls: real model inference requires API keys that were deliberately not present; provider behaviour is proven through scripted gateways at the unit level, and every execution path is proven to fail closed.
  • Deferred: M10 (69 dead modules / 2,910 LOC) and M11 (docs drift) are documented in PHASE0-2_REPORT.md but not yet actioned — large-scale removal needs its own pass.

PHASE0-2_REPORT.md (committed here) contains the full Phase 0–2 deliverable: architecture walk-through, truth audit with every command and output, and the prioritized defect report.

Summary by CodeRabbit

  • New Features

    • API responses now identify degraded results, failed specialists, and coverage, with clearer error details when providers are unavailable.
    • Production deployment instructions now cover a single API service with persistent storage and a health check; environment configuration is optional.
    • Prometheus metrics now report live monitoring data.
  • Security

    • Protected API routes now require authentication, and credential-bearing prompts are checked after sanitization.
    • Rate limiting now supports trusted-proxy client addresses.
  • Bug Fixes

    • Provider failures no longer appear as successful answers; affected requests return degraded or unavailable responses.
    • Timestamps use consistent timezone-aware UTC values, and long-running caches and tracking lists are bounded.

…ty boundaries

Addresses every CRITICAL and HIGH defect from the Phase 0-2 audit, each with a
paired regression test (added in the following commit).

CRITICAL - fabricated results. Total provider failure returned HTTP 200 with
SUCCESS, confidence 0.92, canned evidence and total_tokens:120. Unified
execution now returns status="degraded" with the real error; /v1/ask,
/v1/task/execute, /v1/agent/assist, friday ask and code generation all report
DEGRADED at confidence 0.0 instead of succeeding, and degraded results are
never written to cache.

CRITICAL - 114 of 120 API operations accepted anonymous callers. One
fail-closed dependency (require_inference_api_key) is now applied router-wide:
no server key -> 503, missing credential -> 401, wrong credential -> 403 via
hmac.compare_digest. Health probes, /agents and the dashboard stay public.
X-FRIDAY-API-Key / Authorization parameters that were declared but never read
were removed so OpenAPI no longer advertises credentials it ignores.
verify_friday_api_key now delegates to the same core instead of answering 500
where the other boundary answered 503.

CRITICAL - rotate_tenant_key accepted any old_key (including empty), returned
200 ROTATED and appended an attacker-supplied key to active_keys on every
call. It now raises PermissionError (403) when old_key is not active, mints
with secrets.token_hex(8), and retires the superseded key.

CRITICAL - /v1/governance/prometheus-metrics returned seven hardcoded series
(412/1240/88 requests, every provider health=1) wrapped in JSON a scraper
could not read. It now emits real Prometheus text built from monitor,
concurrency_controller, telemetry_cache and circuit_breaker_manager, with
provider series emitted only once real calls exist.

HIGH - multi-agent debate returned "[Specialist X temporarily offline]" prose
and constant confidence 0.90. Failed specialists now raise AgentCallUnavailable
and are retried through peer candidates with coverage recorded; an all-dark
panel raises AgentPanelUnavailable -> 503. Confidence is earned from extracted
claims/evidence rather than hardcoded.

HIGH - trading consultation recorded failed runs as status="completed" with an
error attached, claimed "Multi-Agent Deliberation" from canned fallback text,
and used a flat 0.88 confidence. Participation now drives confidence
(0.45-0.88), fallback entries are marked, run records are honest, and degraded
decisions are not cached.

HIGH - both rate limiters keyed on request.client.host, collapsing every caller
behind a proxy into one bucket and granting a localhost bypass to a local
proxy. resolve_client_ip trusts X-Forwarded-For only from loopback or
configured proxy CIDRs; the `"pytest" in sys.modules` bypass trigger is gone;
.env.example no longer ships APP_ENV="development"; a malformed Content-Length
no longer raises a 500.

HIGH - memory records mixed naive datetime.utcnow() with aware
datetime.now(timezone.utc), making compare/subtract raise TypeError. All 16
call sites are aware UTC and SQLite normalises legacy naive rows on read.

MEDIUM - detect_credentials ignored 2 of 9 patterns so the audit reported
clean while scrubbing was actively redacting; both are now covered and the
scrub sites verify afterwards, refusing 500 rather than forwarding a secret.
Three process-global maps keyed by caller-supplied values are now bounded.
app/monitoring/alerts.py and app/optimization/token_optimizer.py were
unreachable duplicates of live top-level modules and were removed.

conftest.py provides the shared `auth` fixture that configures a server-side
test credential and returns the matching headers, so authenticated tests
authenticate rather than bypass.

pytest config is consolidated: pytest.ini shadowed pyproject.toml, causing
pytest to warn "ignoring pytest config in pyproject.toml!" and silently drop
asyncio_mode=auto. The two are merged into one [tool.pytest.ini_options]
section (configfile: pyproject.toml, asyncio: mode=Mode.AUTO).

…e audit report

docker-compose.production.yml described an architecture this application does
not implement, and every part of it blocked start-up: nginx/ was bind-mounted
but the directory does not exist, api-1/2/3 published no ports (nginx fronted
them), env_file: .env was mandatory while the repo ships .env.example, and
redis/postgres were provisioned although nothing in app/ connects to them and
no driver for either is declared. The obsolete top-level version key is gone.

The app is one uvicorn process with a SQLite store, so the file now provisions
exactly that: one API publishing 8000:8000, a persistent /app/data volume, a
healthcheck against /health, an optional .env so a fresh clone boots, and
APP_ENV=production.

.dockerignore began with a UTF-8 BOM, making the first pattern literally
"\ufeff.git" - so `.git` was never excluded and full history shipped in the
image. BOM removed and `.venv` added (neither was ignored before).

README gains a "Run It With Docker Compose" section documenting the optional
.env and the fail-closed 503 behaviour without provider keys.

PHASE0-2_REPORT.md is the Phase 0-2 deliverable: what the system is, the
request/execution flow, the truth audit with every command and its output, and
the prioritized CRITICAL/HIGH/MEDIUM/LOW defect report with file, line, root
cause, evidence and proposed fix for each finding.

CAVEAT: docker is not installed in the audit environment, so the compose file
was validated structurally by tests/test_docker_stack.py (11 assertions, 6 of
which fail against the previous file) rather than by executing `compose up`.

📝 Walkthrough

Walkthrough

The pull request changes provider failure responses, multi-agent and trading consultation results, API authentication, request security, runtime state handling, timestamps, metrics, and production deployment configuration. It also adds regression tests and an audit report.

Changes

Inference result integrity

Layer / File(s) Summary
Provider failure signaling
app/providers/unified_manager.py, app/api/*, app/services/code_generation.py, tests/test_no_fabricated_results.py
Provider failures now return degraded results with errors and empty output instead of fabricated success content. Degraded results are not cached.
Multi-agent recovery and degradation metadata
app/agents/debate.py, app/core/orchestrator.py, app/api/routes.py, tests/test_multi_agent_self_healing.py
Failed specialists can be covered by peers. Results carry failure and coverage metadata, and the API returns an unavailable-panel error when no specialist succeeds.
Trading consultation outcomes
app/services/trading_consult_service.py, app/schemas/trading_consult.py, tests/test_trading_consult_honesty.py
Trading consultation records model-backed and fallback outcomes, adjusts confidence based on model participation, and skips caching degraded decisions.

API access and request security

Layer / File(s) Summary
Shared authentication and tenant-key rotation
app/core/security.py, app/main.py, app/api/routes.py, app/governance/tenant_manager.py, app/routers/governance.py, tests/test_api_authentication.py
Protected routers use shared API-key authentication. Tenant key rotation rejects an inactive old key and maps that failure to HTTP 403.
Credential detection and verified scrubbing
app/security/prompt_isolation.py, app/api/v1_core_routes.py, tests/test_credential_isolation.py
Credential detection covers additional secret patterns. The v1 ask and debate routes reject prompts or context that fail sanitization checks.
Proxy-aware client IP and rate limiting
app/core/config.py, app/core/security.py, app/middleware/rate_limiter.py, app/security/api_security.py, tests/test_client_ip_trust.py
Rate limiting uses resolved client IPs and configured proxy trust. Loopback exemptions and malformed request-size handling use explicit checks.

Runtime state and observability

Layer / File(s) Summary
Bounded request and cache state
app/routers/trading.py, app/governance/tenant_manager.py, tests/test_bounded_state.py
Bot request windows and tenant deduplication storage remove stale entries and enforce configured limits.
UTC-aware timestamps
app/memory/base.py, app/memory/sqlite.py, app/services/experiment_service.py, tests/test_datetime_awareness.py
Record defaults and SQLite reads use timezone-aware timestamps. Offset-free stored timestamps are assigned UTC.
Live Prometheus metrics
app/routers/governance.py, tests/test_prometheus_metrics.py
The governance metrics route emits Prometheus text using current monitoring, circuit, provider, and health state.

Deployment and repository maintenance

Layer / File(s) Summary
Production Compose setup and documentation
docker-compose.production.yml, README.md, .env.example, .dockerignore, tests/test_docker_stack.py
The production Compose stack now uses one API service with persistent SQLite storage and an optional environment file. The README documents startup and service endpoints.
Phase audit report
PHASE0-2_REPORT.md
The report records architecture and endpoint observations, audit findings, items classified as non-bugs, and a request to defer later phases pending approval.
Pytest configuration and module cleanup
pyproject.toml, pytest.ini, app/monitoring/alerts.py, app/optimization/token_optimizer.py, tests/test_module_importability.py
Pytest discovery settings move into pyproject.toml, and pytest.ini is removed. The alert and token-optimizer modules are deleted, with tests added for module importability and name collisions.

Merge Risk: 🟠 High · up to fe54d

CI fails during test collection, so the regression suite does not run. A provider outage can still turn a real multi-agent answer into a server error when the panel reports a conflict. Strict scrapers may reject the metrics output. Rate limits can be evaded through eviction or forwarded headers. These issues should be fixed before merge.

Security Architecture Review

Security architecture risk: 🟡 Moderate · up to fe54d

Authentication and failure reporting improve substantially. Remaining risk centers on the directly published HTTP listener and proxy-header trust, whose safety depends on deployment controls that have not been established. A pre-existing memory-exhaustion condition also remains despite tighter request access and state limits.

Retained concerns

  • High · security · inferred: The revised production stack directly publishes an HTTP-capable Uvicorn listener and removes the supplied nginx and SSL configuration. Without external TLS termination and restrictions on direct port access, API credentials and request data can traverse an unencrypted path. This is a new deployment-boundary concern, not proof of an exposed production installation; the prior nginx configuration was also not demonstrably runnable.
  • Low · security · inferred: The new shared resolver automatically trusts loopback peers and accepts the last forwarded value or X-Real-IP without validating it as a client address. Both rate limiters now consume that value. A local caller or trusted proxy preserving attacker-controlled headers can therefore select quota identities; additional proxy hops can instead collapse unrelated callers into one identity. Direct untrusted peers cannot trigger this path, and external exploitability remains dependent on the unavailable ingress contract. The base already exempted loopback in the primary limiter, so not every bypass is newly introduced.
Security review details

Security Blast Radius

  • inferred — A compromised accepted service key reaches multiple protected router families, including model execution and governance, rather than a demonstrably tenant-scoped authority. Memory exhaustion in the trading limiter affects the shared API process. Neither observation establishes compromise of other services, exchange accounts, or environments.

Security Findings and Attack Paths

  • observed — The retained denial-of-service finding concerns caller-supplied bot IDs with no string-length bound. A 1 MiB request limit and 10,000-entry cap can still retain substantial memory; upstream quotas are per client identity, not retained bytes. The unconstrained identifier and retention condition existed at the base. The PR adds authentication and reduces cardinality, so the residual finding is not presented as a demonstrated PR regression; effective deployment exposure remains unresolved.
  • inferred — The retained proxy-trust finding concerns forwarded identities reaching shared quota controls. Exploitation requires a local connection or a trusted forwarding path that permits attacker influence over the selected header. Arbitrary direct remote peers are rejected as trusted hops; deployed CIDRs, header rewriting, and proxy order remain unresolved rather than verified external attack paths.

Trust Boundaries and Controls

  • observed — The normal authentication path fails closed and compares supplied credentials with configured keys using hmac.compare_digest. An explicit INSECURE_DEV_AUTH setting bypasses authentication. The supplied production Compose configuration does not enable that override, but authentication does not itself provide transport encryption.

Resilience and Maintainability Implications

  • observed — The bot limiter prunes idle windows and evicts live least-recently-used windows at capacity before checking quota. A consultation attempt consumes its timestamp before downstream execution, including on subsequent failure. Limits are process-local and eviction resets removed histories, so bounded memory cardinality is not a durable per-principal quota guarantee.

Hardening Proposals

  • proposed — Require an explicit production transport contract: HTTPS termination, blocked direct access to the published backend port, exact trusted proxy networks, and defined forwarded-header rewriting and hop selection.
  • proposed — Bound bot-ID length and aggregate retained bytes, and bind quotas to authenticated principals rather than caller-selected identifiers. If tenant rotation is intended to provide durable credential revocation, add persisted rotation state and a recoverable, caller-bound retry contract.

CI failed: Pytest collection failed in job 112452987794 due to a ModuleNotFoundError when importing tests.conftest, caused by incorrect python path configuration.

Overview

1 change-related test collection failure was found across 1 analyzed log, caused by an absolute import path issue in the newly added test suites.

Failures

Pytest Collection Error (confidence: high)

  • Type: build
  • Affected jobs: 112452987794
  • Related to change: yes
  • Root cause: The test file attempts to import from tests.conftest via absolute import, but python cannot resolve the 'tests' package because PYTHONPATH or the execution method does not include the root package.
  • Suggested fix: Run pytest using 'python -m pytest' or set PYTHONPATH=. in the test runner workflow.

Summary

  • Change-related failures: 1 build/collection failure due to test import resolution.
  • Infrastructure/flaky failures: 0 failures.
  • Recommended action: Update the CI test execution command to run via 'python -m pytest' or configure PYTHONPATH=. so absolute test module imports succeed.

🔴 High risk · Router-wide authentication and tenant-key rotation change access-control behavior.

Fixes 4 CRITICAL and 4 HIGH audit findings across provider failure masking, key rotation, authentication, and metrics reporting, with 165 new regression tests. Three important issues remain open: conflict-path rebuttal/final-resolution calls abort the task without catching AgentCallUnavailable; /v1/debate and FRIDAY routes ignore the degraded flag and cache non-answers; X-Forwarded-For values are used unvalidated and the testclient rate-limit bypass has no environment gate. Additionally, Prometheus output repeats HELP/TYPE lines for one metric family. These must be addressed before merge.

⚠️ Bug: Rebuttal/final-resolution calls now raise and abort the whole task

📄 app/agents/debate.py:724-738 📄 app/api/routes.py:118-130

_execute_agent_call used to return a placeholder string when a call failed. It now raises AgentCallUnavailable. Only Round 1 (analyze_agent) and the first synthesis (debate.py:686) catch it. The conflict path calls the critic rebuttal (debate.py:724) and final_resolution synthesis (debate.py:755) with no guard. So if the synthesizer emits CONFLICT_DETECTED: and the critic's provider is dark, the whole collaboration raises, even though Round 1 and the synthesis already produced real output. /ask and /debate then hit the generic except Exception and return a 500 'Task orchestration failed', not a degraded result or a 503. Fix: catch AgentCallUnavailable around both calls, append to degradation_reasons, and fall back to the existing synthesis_text at capped confidence.

run_collaboration now returns results with degraded=True in two cases: a peer covered a dark specialist, or the synthesizer was dark and raw specialist output was returned unmerged. debate_v1 ignores collab_result.degraded and always returns status="SUCCESS" with failure_state=None. friday_ask (DAG path) and friday_debate drop the degraded/agent_coverage fields entirely. They also store the result in the L1 perf_cache for 180s/300s, so the non-answer keeps being served after the provider recovers. This contradicts the PR's claims that degraded status propagates to every execution route and that degraded results are never cached. Fix: when degraded is true, set status="DEGRADED" and fill failure_state from degradation_reasons in debate_v1; in the FRIDAY routes, expose the flag in provenance and skip perf_cache.set_query.

Many local proxies pass a client-supplied X-Forwarded-For through unchanged, e.g. nginx configured with only proxy_set_header X-Real-IP, or any proxy that does not append. The other tests also do not rely on its monkeypatch, because pytest reverts the monkeypatch after the test. Delete the test, or apply this patch through a fixture that the dependent tests use.

Apply the peer discount to the covered specialist, not to every perspective from that peer.

covered_peer_ids holds peer IDs. r1_results stores the peer Agent as the target of a covering result. Assume a peer is also a panel member and runs its own analysis. In that case, both of its perspectives lose 0.10, although only the covering perspective is second-hand. Also, failed_agents is shared mutable state across concurrent analyze_agent coroutines. A peer that fails while covering is added to failed_agents. That peer's own primary result can still succeed. _degradation_fields then reports the peer as unavailable. Track the covering status for each result. For example, return a covered_for value from _attempt and discount only those results.

Track covering status per result in the `_attempt`/`analyze_agent` flow, such as
by returning a `covered_for` value from `_attempt`, and apply the 0.10
confidence discount only to perspectives produced on behalf of a covered
specialist. Keep a covering peer’s successful primary result undiscounted, and
prevent a covering failure from marking that peer unavailable when its own
analysis succeeded so `_degradation_fields` reports the correct status.

Catch synthesis failure in the conflict path as well.

The Round 2 synthesis now catches AgentCallUnavailable. The rebuttal call to _execute_agent_call and the final-resolution call to _execute_multi_model_synthesis do not catch it. Assume Round 1 succeeds, synthesis reports CONFLICT_DETECTED:, and the critic or synthesizer models then fail. In that state, AgentCallUnavailable propagates to the caller. /ask returns HTTP 500 and discards the real specialist and synthesis output. The stated goal is to return degraded output whenever genuine material exists. Wrap the rebuttal and final-resolution stages in the same way. On failure, set synthesis_degraded = True, add a reason, and return synthesis_text or combined_proposals at the capped confidence.

Also, the degraded-synthesis text is never checked for CONFLICT_DETECTED:, so the fallback cannot trigger the rebuttal path. That behavior is correct.

Require explicit configuration before trusting loopback peers for forwarded headers. TRUST_PROXY_HEADERS defaults to True, and _is_trusted_hop trusts loopback peers unconditionally. If a local proxy passes client-supplied forwarding headers through unchanged, a client can choose the resolved IP and evade rate limits. If a development bypass is enabled, a client can also supply a loopback IP to trigger that bypass. Trust loopback only when explicitly configured, and keep the default untrusted.

Normalize offset-aware timestamps to UTC.

If a caller saves a TaskRecord with created_at at +05:00, save_task stores that offset and _parse_timestamp returns it unchanged. The retrieved timestamp is aware but not UTC, contrary to this helper’s return contract. Return parsed.astimezone(timezone.utc) and cover an offset-aware stored value in the tests.

Document INFERENCE_API_KEY as a requirement for protected endpoints.

The README says model calls fail with 503 until provider keys are configured. That cause is incomplete. In app/core/security.py:94-126, _authenticate raises 503 on every protected endpoint when no integration key is configured. Provider keys alone do not fix this. An operator who follows these steps and sets only GEMINI_API_KEY/GROQ_API_KEY still gets 503 and gets no hint from this section. State that both INFERENCE_API_KEY and at least one provider key are required. State that clients must send the integration key.

Reject duplicate Prometheus metadata.

This test checks sample lines but ignores # HELP and # TYPE lines. The supplied governance handler emits both lines three times for inference_request_duration_seconds, once per quantile. Prometheus permits only one of each per metric name, so this test accepts an invalid response. Assert metadata uniqueness or parse the full response, and emit the quantile samples under one metadata pair. (prometheus.io)
