Reviews now resolve repository-verifiable uncertainty against the exact base and head revisions while preserving diff-only analysis when repository files are unavailable. Unresolved repository questions remain visible without blocking merges, operational anchors stay out of model-facing findings, and GitHub file fallback only occurs for a definitive missing head revision.
