Add a lightweight Web experience feedback form linked to #455. Users describe their goal, actual experience and expected behavior; screenshots and environment are optional. Keep blank issues available and add the tracking discussion to the issue chooser.
This publishes the community intake path; it does not close the ongoing tracking issue or change runtime behavior. Existing Web tasks are already linked under #455, itself a child of #76.
Validation: YAML parsed with Ruby Psych; verified three required fields and retained blank-issue access. Repository check and full tests run separately from GitHub's hosted form rendering.
