chore: ignore local agent and tool working files - #118

- ignore .hyperi-ai (the symlink the slashed form does not match), .superpowers/, .playwright-mcp/, .kube/ and .secrets/ - let every .env*.example file through the .env.* rule - drop 21 repeated patterns and the two truncated comments above them

Local agent and tool state should stay out of git status, and nothing tracked should sit under an ignore rule.
Done when git ls-files -ci --exclude-standard prints nothing.
