feat: add Lulu publication adapter pack

Summary

• add a versioned, offline Lulu publication provider pack with pinned official-source observations
• add independent print-direct, Lulu Bookstore, global distribution, PDF ebook, EPUB distribution, Amazon, Ingram, and Barnes & Noble eligibility decisions
• add fail-closed print/cover/template/proof/barcode/metadata and EPUB 3.3/EPUBCheck/accessibility evidence
• add machine-readable request/report schemas, CLI commands, adapter discovery, documentation, and a 44-page color-comic regression fixture
• preserve the provider-neutral publication contract and perform no login, upload, ISBN allocation, proof order, or publishing action

• JSON schema parse checks
• Lulu rule-pack serialization smoke
