Failed batch reads previously hid successful items, and completed task failures disappeared with short-lived queue history. Preserve bounded failure summaries for seven days, display batch success/failure counts, and clarify revision and MCP source-identity guidance without changing read or search semantics.
Validation: full Go suite, formatting/vet/web checks, production build, browser checks, batch regression tests, archive/clear/lease tests and public-content audit passed. Storage contract coverage includes optional failure metadata; CI verifies PostgreSQL and MongoDB.
