Close tool and effect proof gaps with an eight-case policy tuple

What changed

• Added an eight-case local rule-binding suite for exact or different tool, effect, and resource fields, plus an independently coded Python state oracle and falsification tests.
• Extended source mutation checks to nine selected changes. The 384-vector matrix detects seven; the tuple suite detects the two tool/effect mutations that previously changed zero matrix rows. All nine differ across the combined suites.
• Built an interactive three-field gate with inspectable proposal/rule bindings, receipts, callback counts, and case deep links; connected it to the dispatch atlas and refreshed a stale 192-vector label.

• Focused boundary, mutation, and viewer tests; local link and deployment surface checks
