Merge PR #44 (gap remediation O-01..O-12, ADR-0001) once CI is green #46

Context

PR #44 carries six commits from the September gap remediation pass that sat unpushed on the local OneDrive copy for about six weeks.

Work

1. Wait for CI: integration job has never passed - T&E tables not found in te_dev #45 to be green on gap-remediation-2026-09.
2. Merge with Create a merge commit. Do not squash or rebase: both rewrite commit IDs and leave local main and art-milestone-0 out of step.
3. Sync locally:

Done when

• PR Gap remediation pass O-01..O-12 and ADR-0001 #44 is merged with all checks green.
