Both files documented this convention themselves but the previous release shipped without following it: improvements.md's `## Done` section (resolved items belong in changelog.md + git history + the ADR-0012..0016 decisions instead) and handoff.md's long completed-task narrative are cleared back to their steady-state, current-only form. Nothing in ## Open or Open risks changed in substance.
