Fixes the live TPS label during tool calls and step settlement: the rate is now captured at the authoritative stream-end boundary and held until new bytes arrive, instead of decaying during opaque tool time or being replaced by a run-wide average. Fallback behaviour for hosts without
session.step.streamed, retry handling for reused assistant message IDs, and timer shutdown once the held rate can no longer change are covered by new tracker and plugin tests. Final run accounting and averaging are unchanged.
