feat(alerts): on-call schedules in the dashboard: who is on call, a calendar, overrides, and paging people - #2307
This is the on-call side of the dashboard. You can build schedules, see who is on call, cover a shift with an override, point escalation steps at people and schedules, and see on the incident page who was paged.
What changed
On-call tab in the alerts hub, between SLOs and Routing.
Each schedule row shows the name, time zone, layer count, who is on call until when, and when the next handoff is.
A schedule with nobody on call says so.
The empty state explains what a schedule is and links to create one.
Schedules get their own tab rather than a Routing section: they are about people, and each has its own calendar page.
"On call now" strip on top of the inbox, with each schedule's current person. It refreshes on the hub's existing clock once a shown shift hands off.
Schedule form (/alerts/schedules/new, edit):
name, description, and a searchable time zone defaulting to yours;
up to 5 layers you can reorder;
people in rotation order;
Daily, Weekly or Custom handoff, a start time read in the schedule's zone, and optional weekly windows;
validation mirrors the API.
Schedule page:
who is on call now;
a one-week calendar with a "Final schedule" row over each layer, overrides highlighted and a now-line;
days follow the zone's local clock, so a DST day is 23 or 25 hours;
a note when your zone differs from the schedule's;
clicking a shift (or "Add override") opens an override dialog for that range;
upcoming overrides, with delete.
Chain dialog: each step adds destinations, people and "whoever is on call on " from one menu. Every target kind survives load and save, and the form locks while a save is in flight. The Routing summary names the people and schedules.
Incident timeline: "Paged Ben Brook by Slack DM", with failed and skipped pages saying why. These come from the shared /events activity feed alongside acknowledgements.
Kit:
ShiftTimeline and PersonAvatar/PersonLabel in packages/views/src/ui;
PickList and FormScrollArea in apps/web/src/page/kit.
destinationPicker, the members settings tab, and the SLO and uptime forms moved onto them.

Review fixes
An independent review and the review bot found 15 issues, none high severity. Fixed, each with a test:
Overrides and the calendar:
an override opened from a shift that crosses the visible week keeps the shift's real bounds;
deleting an override asks first, and a failed delete refreshes the list;
week navigation shows a skeleton, not an empty calendar;
a failed read stays on screen with Try again instead of retrying every tick.
Schedule page header: it refreshes at a handoff and after override writes.
Members:
members load past 100, up to 2,000;
an unknown or not-yet-loaded member reads "Member", and only a known departure reads "A former member".
Schedule form:
it lists the top layer first, so "Move up" raises a layer;
a saved start (seconds, or the repeated 02:00 hour on 2026-10-25) round-trips unchanged unless edited.
Timeline accessibility: bars are focusable, the header has proper roles, and short shifts have a minimum width.
Fetching:
the hub reads members only on tabs that need them;
An open incident can now be acknowledged ("someone is on it") and
unacknowledged. The ack is a sub-state of `open`, like the telemetry hold:
four nullable columns on alert_incidents (who, when, name, source), so every
open/resolved consumer keeps working. alert_incident_events keeps the history
(acknowledged, unacknowledged, ack_cleared) for the timeline and for on-call
escalation to read.
The lifecycle planner holds reminders and de-escalation notices while an
incident is acknowledged. An escalation to a higher severity still pages and
clears the ack. Resolve still sends the all-clear and clears it.
Surfaces: POST /v2/alerts/incidents/:id/acknowledge and /unacknowledge
(alerts:write, 409 on a resolved incident), GET /:id/events, an
acknowledged=true|false list filter, and the acknowledge_alert_incident MCP
tool (approval-gated in chat like the other mutating tools).
… in SQL
The escalation cleared the ack from the tick's prefetch and wrote ack_cleared
after the claimed update, outside it. A failed event insert could then lose
the escalation page, and a human ack or unack during the evaluation got the
wrong outcome. Every escalation now clears whatever ack stands when it
commits, with a guarded update in the same claimed write, and writes
ack_cleared only when one was cleared. The planner no longer carries the
flag.
Also: an API-key ack records no user (the key is not a person), the
unacknowledge docs say an overdue reminder goes out on the next check, and
the MCP incident list takes an acknowledged filter.
Domain schemas for on-call schedules (layers, rotations, restrictions, overrides) and a pure resolver: onCallAt and renderOnCall with DST-aware local handoffs.
The incident document gains a nullable escalation object (chain, status, next step, pass,
next page time) read from alert_escalation_runs in one batched query per page.
… and lose to a resolve or ack
- the scheduler reads only runs with a decision to make, so standing acks cannot crowd out due steps
- a step locks its incident and the drain drops a step whose incident closed or was acknowledged
- a flap-suppressed re-open starts the chain past step 1 instead of dropping it
- de-escalation stops the critical tier's chain; escalation refreshes a running chain's payload
- v2 PATCH tiers without escalation_chain_id keep the stored chain; MCP "none" detaches every tier
Schedules and overrides as org-scoped tables, and OnCallSchedulesService: CRUD, overrides, who is on call at an instant and rendered shifts. Participants and override users must be workspace members; 50 schedules per org, 500 upcoming overrides per schedule, overrides at most 90 days and not over already.
/v2/alerts/oncall_schedules CRUD with onc_ ids, who is on call at an instant, rendered shifts and ovr_ overrides, audited. MCP tools list_oncall_schedules, who_is_on_call, create/update/delete_oncall_schedule and create/delete_oncall_override, naming people by display name.
…build them
Each chain reads as its steps in one line and the rules that start it. The dialog edits
name, ordered steps (destinations and a wait), and repeats, with the API's limits checked
before save. A refused delete names the rules still holding the chain.

The open McpQueryError issues sit in page/registry.ts frames this diff only touches to register a lazy page, so it does not address them.
