feat(alerts): build escalation chains in the dashboard and see them on incidents - #2305
Routing tab, "Escalation chains" section. Each row shows the name, a one-line step summary ("Slack #alerts-api, then PagerDuty after 10m, ...; repeats twice"), how many rules use the chain, and an edit/delete menu. The empty state explains what a chain is and offers "Create chain"; there is no demo data. A delete that the API refuses with 409 names the rules still using the chain, inside the dialog.
Chain dialog. Name, description and ordered steps. Each step has a destination picker and a wait in minutes, and steps can be moved up, moved down or removed; the chain also has a repeat count. It checks the same limits as the API schema before saving, and shows save errors inside the dialog.
Rule editor. Each tier has an "Escalation chain" picker: "None", the org's chains, and a "Create a chain" link that opens in a new tab so the draft isn't lost. A tier needs a destination or a chain, matching the server.
Incident page. An Escalation section at the top of the rail shows escalating, paused (acknowledged) or finished, with "step 2 of 3, next page at 12:10". It shows "paused" as soon as the incident is acknowledged, before the scheduler gets to it.
v2: alert incidents carry a nullable escalation object built from the latest run: chain, status, step, pass, steps sent, and next step time. A single incident costs one query; a list costs one batched query per page. The iOS OpenAPI spec and the contract tests are updated.
Lab:/lab/escalation?state= has 10 states, used for the screenshots below.
No kit components were added. Everything uses the existing destination picker, form and confirm dialogs, select, row menu, typography and stack.

These sections are new: none of these surfaces existed before, so there is no "before" image except the rule editor, which only gains the picker.

New tests:
scene tests: hub-chains (empty, populated, member view, dialog validation, delete refused), editor-chains (tier picker validation), incident-escalation (escalating, paused, finished, and when it reloads);
unit tests for the step summary and the dialog;
a v2 test for incident escalation (latest run on get and list, null without one, paused).

New Features
Manage escalation chains from the Alerts Routing tab: create, edit, and delete chains with ordered notification steps, wait times, and repeat settings.
View chain summaries and associated alert rules, with clear loading, empty, and retry states.
Assign escalation chains to warning and critical alert tiers alongside notification destinations.
View an incident’s escalation status, chain details, sent steps, and next scheduled step, with updates as escalation progresses.
Incident responses now include escalation details when available.
Bug Fixes
Acknowledged incidents show an acknowledged escalation state without a scheduled next step.
Alert rules with escalation chains no longer receive an extra default notification destination.
An open incident can now be acknowledged ("someone is on it") and
unacknowledged. The ack is a sub-state of `open`, like the telemetry hold:
four nullable columns on alert_incidents (who, when, name, source), so every
open/resolved consumer keeps working. alert_incident_events keeps the history
(acknowledged, unacknowledged, ack_cleared) for the timeline and for on-call
escalation to read.
The lifecycle planner holds reminders and de-escalation notices while an
incident is acknowledged. An escalation to a higher severity still pages and
clears the ack. Resolve still sends the all-clear and clears it.
Surfaces: POST /v2/alerts/incidents/:id/acknowledge and /unacknowledge
(alerts:write, 409 on a resolved incident), GET /:id/events, an
acknowledged=true|false list filter, and the acknowledge_alert_incident MCP
tool (approval-gated in chat like the other mutating tools).
… in SQL
The escalation cleared the ack from the tick's prefetch and wrote ack_cleared
after the claimed update, outside it. A failed event insert could then lose
the escalation page, and a human ack or unack during the evaluation got the
wrong outcome. Every escalation now clears whatever ack stands when it
commits, with a guarded update in the same claimed write, and writes
ack_cleared only when one was cleared. The planner no longer carries the
flag.
Also: an API-key ack records no user (the key is not a person), the
unacknowledge docs say an overdue reminder goes out on the next check, and
the MCP incident list takes an acknowledged filter.
Domain schemas for on-call schedules (layers, rotations, restrictions, overrides) and a pure resolver: onCallAt and renderOnCall with DST-aware local handoffs.
The incident document gains a nullable escalation object (chain, status, next step, pass,
next page time) read from alert_escalation_runs in one batched query per page.
… and lose to a resolve or ack
- the scheduler reads only runs with a decision to make, so standing acks cannot crowd out due steps
- a step locks its incident and the drain drops a step whose incident closed or was acknowledged
- a flap-suppressed re-open starts the chain past step 1 instead of dropping it
- de-escalation stops the critical tier's chain; escalation refreshes a running chain's payload
- v2 PATCH tiers without escalation_chain_id keep the stored chain; MCP "none" detaches every tier
Schedules and overrides as org-scoped tables, and OnCallSchedulesService: CRUD, overrides, who is on call at an instant and rendered shifts. Participants and override users must be workspace members; 50 schedules per org, 500 upcoming overrides per schedule, overrides at most 90 days and not over already.
/v2/alerts/oncall_schedules CRUD with onc_ ids, who is on call at an instant, rendered shifts and ovr_ overrides, audited. MCP tools list_oncall_schedules, who_is_on_call, create/update/delete_oncall_schedule and create/delete_oncall_override, naming people by display name.
…build them
Each chain reads as its steps in one line and the rules that start it. The dialog edits
name, ordered steps (destinations and a wait), and repeats, with the API's limits checked
before save. A refused delete names the rules still holding the chain.

Test write filtering in the hub and editor wrappers.
The incident assertions exercise their filter, but they do not cover chain saves, chain deletes, or rule saves. Add tests for those writes through GotHubMessage and GotEditorMessage. Use states that produce write commands before filtering so the assertions cannot pass because of a guard.

Adds escalation chains to the dashboard: a Routing-tab section with create/edit/delete, a per-tier picker in the rule editor, an escalation block on the incident rail, and a nullable escalation on v2 alert incidents filled from one batched run query per page. Safe to merge.
listIncidentEscalations reads a page's latest runs in one query and v2 incidents expose escalation
Routing tab gains the chains section, the chain dialog and a 409 delete naming its blockers
Rule editor tiers get an escalation-chain picker beside their destinations
Incident rail shows escalating, paused or finished, re-read once a step is due
