Create basic request form

Basic request form requirements:

• Name and Email should come from GitHub (both required)
• email is editable (hint saying required to receive invite?)
• Comment (tell us about yourself / why you should be invited / who referred you) (optional)
• Submit form to Slack webhook with some basic GitHub details such as:
  • Link to GitHub profile
  • public repositories
  • following
  • followers
  • bio
  • location
  • login
  • blog
  • type
  • company
  • public gists

Language selections will be a future enhancement.
